using and disclosing personal information responsibly
for the services we provide. This includes our therapists,
support staff persons, financial institution, consultants
and the agencies we use to provide high quality service
to you
WHAT IS PERSONAL INFORMATION?
Personal information identifies you specifically such
as name, age, home address or phone number, ethnic
backgrounds or health particulars. This also includes
health history, conditions, and services received
or activities and views such as religion, politics,
opinion expressed and opinions or evaluations of an
individual. Business information would be an individual
business address and telephone number which is not
protected by privacy legislation.
1. ACCOUNTABILITY
Under the guidance of our Information Officer, we
are responsible for maintaining and protecting the
personal information we collect, use and disclose.
2. PURPOSES
We collect, use and disclose personal information
in order to provide massage therapy treatments. For
example, we collect information about a client’s
health history in order to help us assess what their
health needs are, to advise them of their options,
to obtain a baseline of health so that in providing
ongoing health services, we can identify changes that
are occurring over time, to give notice or change
of appointment
3. CONSENT
We ask for and receive your consent to collect,
use or disclose your personal information. The only
exceptions would be in cases where use or disclosure
is required or permitted by law, for example in the
course of a criminal investigation or medical emergency.
4. LIMITING COLLECTION
We only collect information for the purposes mentioned
above. This information may be shared by all massage
therapists in the clinic who treat you to avoid duplication
of data collection. We use information such as addresses
and phone numbers to make introductory calls about
our services, to set up appointments, to follow up
on treatments, to discuss therapy options and to provide
information on any major business changes. We do not
mail flyers, or distribute name lists to any other
business and, at present; we do not use e-mail unless
it is requested.
5. LIMITING USE DISCLOSURE AND
RETERNTION
We keep our client files for ten years. Our client
contact directories are much more difficult to systematically
destroy so we remove such information as we can. We
destroy personal files by shredding.
6 ACCURACY
We take steps to ensure your information is accurate,
complete and up-to-date.
7. SAFEGUARDS
Paper information is kept within the staff area. Paper
information is transmitted through sealed addressed
envelopes, hand delivered or sent by reputable companies.
Staff collect, use and disclose personal information
only as necessary to fulfill their duties and in accordance
with our privacy policy.
8. OPENNESS
Information is available in accordance with our privacy
pamphlet and through our Information Officer
9. INDIVIDUAL ACCESS
With only a few exceptions, you have the right to
see what personal information we hold about you if
you ask. We will help with any information you do
not understand. However we will need to confirm you
identity and we reserve the right to charge a nominal
fee. If you believe there is a mistake in the factual
information, you have the right to ask for it to be
corrected. We may ask for documentation as proof that
our files are wrong.

Massage Therapists are regulated by the College of
Massage Therapists of Ontario who may inspect our
records and interview our staff as part of their regulatory
activities in the public interest. If you have any
concerns about our professionalism or competence please
discuss it with us or you are entitled to contact
our regulatory body.
